The Medical Social Work Unit is dedicated to providing patient-centered psychosocial support to individuals and families affected by illness, injury, and disability. Through a multidisciplinary approach, we address the social, emotional, and economic challenges that may impact recovery and overall well-being.
We focus on enhancing coping mechanisms, facilitating access to healthcare services and community resources, and promoting holistic recovery—ensuring continuity of care from hospitalization to community reintegration.
Core Service Programs
Psychosocial Assessment & Counseling
Provides emotional support and counseling to patients and families dealing with trauma, chronic illness, or disability, helping them cope and adjust to life changes.
Discharge Planning & Case Management
Coordinates patient care plans to ensure smooth transition from hospital to home or other care settings, including follow-up services and support systems.
Community Linkages & Resource Mobilization
Connects patients with community resources, support groups, rehabilitation services, and financial assistance programs to support recovery and independence.
Family Support & Education
Engages and educates families and caregivers on patient care, coping strategies, and available support systems for long-term care.
Financial Assistance and Benefits Guidance
Assisting patients in accessing government medical fee waivers, disability grants, insurance claims, and charitable funding to reduce financial barriers to care.
